CARE Team/Situations of Concern

Use the Incident Reporting form to report the following:

  • Concerns about a student's well-being, health, or safety
  • Threats of self-harm, suicide, or harm to others
  • Concerning or escalating behavior that may require intervention
  • Students who may benefit from CARE Team support and coordinated resources

Submit a CARE referral if you are concerned about a student's well-being, even if you are unsure whether intervention is needed.

Safety and Security Reporting

Submit a Safety & Security report to notify Campus Safety of criminal activity, suspicious behavior, security concerns, property crimes, emergencies, or other incidents that may impact the safety and security of the campus community.

Use the Safety and Security Statement form to report the following:

  • Suspicious person, vehicle, or activity
  • Theft, vandalism, or property damage
  • Assault, threats, or weapons concerns
  • Unauthorized access or trespassing
  • Fire, hazardous condition, or emergency
  • Security or access control issue
  • Missing person or welfare concern
  • Any other safety or security concern requiring Campus Safety response

Accident, Illness, or Injury

Use the Accident, Illness, or Injury Form for reporting the following work-related incidents:

  • Slips, trips, and falls
  • Workplace or student injuries
  • Medical emergencies
  • Exposure incidents
  • Vehicle accidents on campus

Bias Incident/Hate Crime Reporting

Use the Bias Incident/Hate Crime Reporting Form to report the following:

  • Bias Incident- An act or behavior that demeans, intimidates, and threatens individuals, members of a group, and a community.
  • Hate Crime - A criminal offense against a person or property motivated in whole or in part by the offender’s biases.

Submit a Bias Incident / Hate Crime referral if you have experienced, witnessed, or become aware of conduct that may be motivated by actual or perceived race, color, national origin, religion, sex, gender identity or expression, sexual orientation, disability, age, veteran status, or another protected characteristic.

Reports may involve conduct that violates college policy, state or federal law, or conduct that is offensive or concerning but may not rise to the level of a policy violation or crime. The College will review each report and determine the appropriate response, resources, and resolution process.

Title IX Violations

Submit a Title IX report to notify the College of concerns involving sex discrimination, sexual harassment, sexual assault, dating violence, domestic violence, stalking, retaliation, or pregnancy and related conditions. Reports may be submitted by anyone who experiences, witnesses, or becomes aware of potential Title IX concerns.

Use the Sexual Misconduct Reporting Form to report the following:

  • Sexual harassment or unwelcome sexual conduct
  • Sexual assault or non-consensual sexual contact
  • Dating violence, domestic violence, or stalking
  • Sex discrimination or pregnancy-related concerns
  • Retaliation for reporting or participating in a Title IX process
  • Pregnancy discrimination or denial of equal access
  • Any other concern involving sex discrimination, sexual misconduct, or retaliation
