Appeal To Go Beyond the Financial Aid Maximum Time Frame (MTF)

What you need to complete this form:
- Financial Aid Academic Plan approved and signed by your advisor
- Contact Academic Advising and set up an appointment.
- Complete the Appeal to go Beyond the Financial Aid Maximum Time Frame (MTF) form
- Attach your Financial Aid Academic Plan signed by your advisor.

- Master Promissory Note (MPN)
Please note: You do not need to submit a copy of the Master Promissory Note or Entrance Loan Counseling. Our Loan officer is able to verify if you have completed them when we are reviewing your Loan Request form.

by going to "Notices/Letters" tab, going to "All Letters" (if no unread letters),
find Monetary Determination letter.

Attending Multiple Colleges
Consortium Agreement
What you need to complete this form:
Important: Contact Financial Aid and explain your situation before you plan on attending multiple colleges.
The deadline for Winter quarter (January 17) has passed.
Begin this process as soon as possible, as the deadline to submit the form is the 1st day of the quarter. If you submit the form after the deadline, you are expected to pay the tuition and fees for the other school while we review your form.
- Complete the form for the appropriate school year:

- Meet with an academic advisor and get their signature on the form. Set up an appointment with an academic advisor.
- Send the Consortium Agreement form to the other college's financial aid department to fill out their section.
- The other college's financial aid department will then send your Consortium Agreement to us and we will review it.

High School Diploma/Transcript, Secondary School Diploma/Transcript, and GED must show your full name and date of graduation.
Note: If your documents are not in English, please have them translated before submitting.
Our department does not need official transcripts.
Unofficial transcripts are accepted.
U.S. Passport, U.S. Passport Card, Permanent Resident Cards, Certificate of Naturalization, Birth Certificate, and I-94: Arrival/Departure Record are acceptable.
Some of these forms have a signature line. Please sign these documents, since unsigned documents are not considered valid.
Attention: Due to a recent change in regulations, some tax transcript requests have been waived by the Department of Education. Please submit a support request so we can verify if you need to provide us with a copy.
What tax year do I need to provide?
The tax year we request is the same year that you provided on your FAFSA or WASFA,
which is two years before the start of the school year, or what we refer to as the
"prior prior" year.
Example: For the start of 2022-2023 school year ...

- The prior year would be 2021.
- The prior prior year would be 2020.
So for the 2022-2023 school year, we would request tax transcripts from the year 2020.

What if I do not have a copy of my tax transcripts for the requested year?
If you do not have a copy of your tax transcripts, you can request a PDF copy from
IRS.gov. You may need to create a profile first if this is your first time requesting a copy
of your tax transcripts.
Alternatively, you can provide us a copy of your signed tax return [1040] or signed amended tax return [1040X] from the requested year. If not signed, we will not be able to accept it.
You can request a letter of your verification of non-filing by visiting IRS.gov.
