2023 - 2024 Financial Aid Program and Enrollment Confirmation
This form is required for all students. Use this form to:
- Confirm the Program of Study you plan to work on while attending Edmonds College
- Tell us the number of credits you plan to take each quarter
2023-24 Adjust Aid Due to Change in Enrollment Level
Use this form when you must change the number of credits you initially told us you planned to take. We cannot disburse financial aid to you if the number of credits you enroll for is different than what your offer of aid is based on.

If you are a first time loan borrower you must also complete the:
Verification forms are divided into type and dependency status based on the FAFSA.
Use this Consortium Agreement Form if you plan to attend another college in the state’s community and technical colleges system, and you plan to use the credits you enroll in at the other school towards the total number of credits needed for financial aid at Edmonds College.

Appeal To Go Beyond the Maximum Time Frame

- Financial Aid Academic Plan approved and signed by your advisor
- Contact Academic Advising and set up an appointment.
- Complete the Appeal to go Beyond the Financial Aid Maximum Time Frame (MTF) form
- Attach your Financial Aid Academic Plan signed by your advisor.
Deadline for appeals is are the first day of the quarter.

Direct Student Loan Request
Disclaimer: Loan Request is for FAFSA students only - Must have completed a FAFSA. See eligibility requirements and basic details about Direct Student Loans here.
Final date to submit Loan Request Form for 2022-2023: June 1
What you need to complete this form:
- Complete online at student aid.gov (the same place you filled out the FAFSA).
- Entrance Loan Counseling
- Master Promissory Note (MPN)
Please note: You do not need to submit a copy of the Master Promissory Note or Entrance Loan Counseling. Our Loan officer is able to verify if you have completed them when we are reviewing your Loan Request form.

Loan Discharge Acknowledgment
Why would I need this form?
If you have had a previous federal education loan discharged due to Total and Permanent Disability (TPD), you may, under certain circumstances, borrow a new federal student loan. See form below for details.
What you need to complete this form:
- Complete the form for the appropriate school year:
- 2022-2023
- 2023-2024 (coming soon)
- You may need a signature from your medical doctor. See form for details.
V4 - Custom Verification
Complete the form for the appropriate school year:
The form needs to be signed and dated. Dependents need their parent or guardian to sign as well.
2. High school diploma or proof of high school completion.
- See form for list of acceptable documents.
- If your documents are not in English, please have them translated before submitting.
3. Proof of Identification: Driver's License, State License, or a U.S. Passport.
4. Submit your documents online!
Note: Starting 2023-2024, the V4-Custom Verification form will need to be signed in person in front of a Financial Aid Administrator or be notarized and sent to our office along with the requested documents.
V5 - Aggregate Verification
What you need to complete this form:
1. Complete the form for the appropriate school year:
This form needs to be signed and dated. Dependents needs their parent or guardian to sign as well.
2. High school diploma or proof of high school completion.
- See form for list of acceptable documents.
- If your documents are not in English, please have them translated before submitting.
3. Proof of Identification: Driver's License, State License, or a U.S. Passport.
