Financial Aid Services Forms
Once we receive your FAFSA or WASFA, you will see any items that need to be completed in your Tasks Tile To-Do list. We will also notify you by email to review your to-do list for any additional documents that are needed. Please do not respond directly to that email. Please use the following ways to communicate with us: Contact us. In some cases, once we start processing your file, we may need more information and will notify you if we need additional documents
Deadlines are early, so make sure you submit all documents listed right away.
IMPORTANT: Fill out forms for the correct academic year.

The online document submission will allow you to attach documents and submit them to our office. Once the Financial Aid form has been completed, you will need to save it as a .pdf to submit them online with any other additional documents. You may need to download a free pdf signature filler online before you complete the form so you can electronically sign the document. Google a signature filler and download it to your computer. You then navigate back to the form, fill it out, save it to your computer, upload to your signature filler, save it again. Try https://smallpdf.com/sign-pdf to sign documents after you have saved them to your computer.
You will receive a automatic notification that the documents were sent to us (check that box at bottom of submission page). Please allow several days for documents to be removed from the Task Tile. Do not submit the same documents multiple times, as the tracking of these is manual and it takes us several days during high volume times (the week before through 3 weeks into the quarter) and is not immediate. Keep your confirmation page email you receive from the document upload for your records. If you are concerned that we did not receive your documents you can submit a support request and we can check on that for you. Faxes and mail may take longer to process, since this is a manual process.

*If updating after the quarter starts, contact Enrollment Services directly, to ask if they can expedite the update in ctcLink, so we may move you forward in the awarding process.

High School Diploma/Transcript and GED proof, MUST show full name and date of graduation.
Proof of Citizenship - Only the following documents are acceptable for proof of U.S. Citizenship:
-A Certificate of Naturalization-Must be signed to be accepted.

-Your Birth Certificate that shows you were born in the U.S
- U.S. Passport (MUST be signed) or a wallet-sized passport card. (Current or Expired is acceptable
Tax Returns (1040 and amended (1040X) MUST be signed to be accepted.
Tax Return Transcripts and Verification of Non-filing Letters are available at irs.gov make sure to select the right year requested. i.e. 2021-22 = 2019 and 2022-23 = 2020 and 2023-24 = 2021.
